The Social Inclusion-Based Library Transformation Program is a national initiative to reposition public libraries as inclusive, participatory, and welfare-oriented community empowerment spaces. This study evaluates the program’s contribution to achieving the SDGs in Indonesia by analyzing five aspects: input, activities, output, outcome, and impact. Using a quantitative descriptive method, questionnaires were distributed to beneficiaries across various regions. Data were analyzed using frequency distributions and percentages. Results show that input indicators resources, facilities, personnel, materials, and implementation timing received generally positive assessments, with an average agreement rate of 65%. program activities were rated highly (78%), particularly regarding diversity, interactivity, flexibility, and comfort. Outputs were strong, with 75% of respondents reporting increased knowledge, skills, access to learning resources, and welfare support. Outcome findings indicate positive changes such as increased confidence, skill application, and social awareness, though SDG-related awareness and economic empowerment require improvement. The impact dimension shows limited long-term contribution to SDGs, with 62% acknowledging benefits related to poverty reduction, education, and environmental awareness. Overall, the program is effective in implementation and immediate outputs, but stronger sustainability strategies and cross-sector collaboration are needed to enhance measurable long-term SDG impact.
